Ask the facility for a written incident report and how it happened. Note who was supervising, the time, and what the facility says.
Write down what you learn
Record what your child, staff, and any witnesses say while it is fresh, and keep any messages from the facility. Photograph any visible injuries and the location if you can.
Ask about staffing and licensing
Note the facility's name and license, and, if you can, how many staff and children were present. This matters for a supervision or ratio question.
