Food Poisoning Checklist

How to Document Your Illness

Connecting your illness clearly to the contaminated food, and capturing its full effect, protects your claim. Here is how.

  1. Get and keep the diagnosis

Get a medical diagnosis and, if possible, lab confirmation of the pathogen, and keep all records. This is central proof.

  2. Keep every record and bill

Save records, test results, prescriptions, and bills, and ask for copies as you go, including any hospitalization.

  3. Track the timeline of symptoms

Note when symptoms started and how they progressed. The timing helps connect the illness to the source meal or product.

  4. Note lasting effects

Keep a private record of any lasting digestive, kidney, or other problems in the weeks after — serious foodborne illness can have lasting complications.

  5. Track lost work and costs

Record missed work, lost income, and out-of-pocket costs. These losses are part of the claim.

The deadlines and rules depend on your state

The steps above apply wherever you were hurt. Filing deadlines, fault rules, and limits on damages differ by state — pick yours for what applies to food poisoning.
