How to Document Injuries and Conditions
Careful documentation over time is some of the most powerful evidence in a neglect case. This checklist helps you do it well.
Photograph injuries with dates
Take clear, dated photos of bruises, bedsores, and other injuries, and keep photographing as they change. A record over time shows how conditions developed.
Note what you are told
Write down staff explanations, who said what, and when. Note whether explanations change or do not match the injuries.
Track the decline
Record weight, mobility, mood, and any decline over time, along with dates. Patterns matter.
Keep all records
Save discharge papers, care plans, bills, and any communications with the facility.
Report and keep the reports
Keep copies of any complaints you filed and any responses, which document that the facility was on notice.
