Slip and Fall Scene Evidence Checklist
Evidence in a fall case vanishes fast — the hazard gets cleaned up and video gets overwritten. If you are able, this checklist helps you capture what matters.
Photograph the hazard
Close-ups of the exact condition — the spill, the broken step, the ice, the obstruction — plus wide shots showing the area and any missing warning signs. Include something for scale if you can.
Document the location and time
Note exactly where you fell, the date and time, and the conditions (lighting, weather), while it is fresh.
Get the incident report and staff names
Ask for a copy of the written incident report and note the names and roles of the staff you dealt with.
Witnesses
Names and phone numbers of anyone who saw the fall or the hazard, including other customers.
Note surveillance cameras
Look for cameras that may have captured the fall or the hazard, and note where they are so the footage can be requested before it is overwritten.
