App and trip records
Screenshot your trip: the driver's name and photo, the vehicle and plate, the pickup and drop-off, the time, and the fare. This proves the driver was working and helps establish app status.
Every driver involved
Name, phone, address, driver's license, plate, and insurance for every driver, and note which one was the rideshare driver.
Photos and video
Wide shots of the scene plus close-ups of damage, plates, the road, signals, and your injuries.
Witnesses
Names and phone numbers of anyone who saw the crash, including other passengers.
Official response
Get the responding officer's name and the crash report number, and note any citations issued.
